In Caerphilly County Borough Council area there are a variety of organisations that can offer support, advice and encouragement to assist people to improve their health and well being.

Caerphilly CB Crossroads

This is a national organisation providing support to carers.  The main aim is to relieve the stresses experienced by carers of people with mental health problems who have care needs.

Food Standard Agency
The FSA provides advice and information to the public and Government on food safety from farm to fork, nutrition and diet. Growing Space

This project aims to relieve sickness by the provision of therapeutic training and work experience for people who have suffered or are suffering from mental illness.

Health Alliance

Caerphilly Health Alliance is a partnership of organisations working together to improve the health and well being of all residents of the Caerphilly borough. It’s work priorities are centred around six themes of Tabacco Control, Obesity, Accidents and Injuries, Alcohol and other Substance Misuse, Infections and Mental Health and Well being.

Healthy Living Centres

Healthy Living Centres provide access to classes and activities that encourage and help people to lead healthy lifestyles.  Six centres are now open in the Upper Rhymney Valley and one in Caerphilly.

Services and activities include healthy cooking and eating, exercise classes, walking or cycling, help to give up smoking, basic computers/internet, help with arthritis, looking after your teeth, advice / help with alcohol and drug problems, arts and crafts, home safety service and community transport.

This is a multi-disciplinary team of health care professionals and Social Services support staff aiming to increase safety and independence in your own home. Reablement intervention is for a period of up to 6 weeks.

Rhymni Valley Mind Step-In

The Step-In Centre welcomes people with mental health issues for a chat and a cuppa.  They have a “nearly new” clothes shop, a one-to-one befriending scheme, trips and outings, subsidised caravanning holidays and a small unit of supported accommodation.

Over 75 Screening Project - Caerphilly Borough
This is a District Nursing service, which delivers a key part of the National Service Framework for Older People by offering all residents of the Caerphilly Borough, aged 75 and over, a health and wellbeing check in their own homes by a qualified nurse.
The aims of Screening are to provide early detection and prevention of disease, monitoring of chronic disease, reducing accidents by simple interventions, improving quality of life and also helping to maintain  independence.

BACKGROUND

Caerphilly Local Public Health Team is part of the National Public Health Service for Wales (NPHS). The aim of the Local Public Health Team is to facilitate the protection and improvement of the health and wellbeing of the people of Caerphilly County Borough, through the delivery of high quality, accessible and appropriate specialist public health services. The department’s programme of work is also designed to implement the Welsh Assembly Government public health policy. This is achieved by working strategically in partnership with local organisations such as the local authority, voluntary sector, NHS Trust and Caerphilly local health board,
 
HOW THE SERVICE FUNCTIONS & WHAT IT PROVIDES:
Caerphilly local public health team is a multi-disciplinary team which includes health promotion specialists and practitioners, specialist nurses and dietitians.  The team works in partnership on a wide range of specific health promotion and public health initiatives, including work on the following areas:

  • Coronary Heart Disease (primary & secondary prevention)
  • Physical activity
  • Mental Health

EXAMPLES OF THE TYPES OF ACTIVITIES / PROGRAMMES OF WORK FOR OLDER PEOPLE
 
  • Moving more often
Moving more often is a programme to promote physical activity with frailer older people.  It is a training programme for health and care workers and volunteers who work with older people and who wish to promote physical activity and learning in a range of care settings.
 
  • Healthy hearts project
Patients are screened for risk of coronary heart disease by the public health primary prevention nurses. The public health dietitian provides educational support for patients who are deemed at high risk of coronary heart disease. Again regular monitoring & evaluation is conducted to ensure the content of the course is relevant to the patients.
 
  • Coronary heart disease  secondary prevention
This initiative is led by clinical nurse specialists who work in primary care to develop and establish coronary heart disease registers and support clinics for patients who have established coronary heart disease. Patients are also referred to the public health dietician, exercise specialist and smoking cessation specialist.
 
  • Fruit & vegetable co-ops
This initiative is led and run by the community members at various locations throughout the borough. The public health dietitian and communities first officer provide support to further develop the scheme.  
 
  • Flu and pneumococcal vaccine
Pneumococcal campaign is year round and people that reach the age of 65 or are diagnosed with a chronic illness are advised to have their vaccine at their GP. 
 
  • Inspire: respiratory project
This initiative is lead by 2 clinical nurse specialists. The aim is to develop  community based chronic obstructive pulmonary disease rehabilitation clinics for people within the community who suffer with this condition, manage case loads of patients who are repeatedly admitted to hospital and  to provide a breathe easy support group.
 
  • Keep well this winter
This Welsh Assembly Government campaign is co-ordinated through the public health team in partnership with local organisation to provide information, advice and support for 65’s and over to warm and well during the winter months. Three key themes of KWTW are: keep warm, keep well and keep safe. The campaign runs from October to February every year.
